  24. Course Description: This course focuses on the collection of subjective data during the interview, analysis of objective data by physical examination of each body system, and the synthesis of the information to formulate, prioritize, and plan nursing interventions.

  26. What is the need or demand for this course? (Indicate if this course is part of a required sequence in the major.) What other programs would this course service? Health Assessment is critical and considered the first step in the process of individualized nursing care. It provides information that is crucial to the development of a plan of action that enhances personal health status. With this information, nurses and other health care professionals are able to establish baseline data and the patient’s current health condition before providing care.

  28. What qualifications for training and/or experience are necessary to teach this course? (List minimum qualifications for the instructor.) The instructor must possess a BSN at minimum, with current clinical experience of at least 32 hours per month.
  29. Objectives: 1.Critically analyze findings from the collection of subjective and objective data and to distinguish between the data consistent with health, and the data which indicates alteration in health.

    2.Obtain a health assessment using proficient interviewing skills.

    3.Conduct a nursing health history, which includes the systematic collection of subjective data to determine health status including collection of physiological, psychological, socio cultural, developmental, and spiritual data.

    4.Identify nursing diagnoses by using data obtained in the assessment.

    5.Develop a therapeutic plan of care identifying outcomes and specific nursing measures that include resources to promote and maintain health.

    6.Complete a physical assessment using: inspection, auscultation, palpation, and percussion.

    7.Communicate a therapeutic plan of care to others.

  34. Student Expectations/Requirements and Grading Policy: Electronic Communication:

    Students are required to have a Suffolk County Community College Email account. Please communicate with course faculty via e-mail frequently.

    Please permit 48 hours for response to emails

    Lab/Clinical Expectations:

    All students are expected to prepare for and participate in ALL clinical sessions. If emergent

    situations arise, directly notify the clinical faculty prior to the start of the clinical day.

    Unreported lateness or absence will result in an “unsatisfactory” grade for the clinical day.

    Student must pass clinical requirements by the end of the semester in order to pass the

    course. All lab absences must be made up.

    WITHDRAWL POLICY:

    Withdrawals are limited to one per course, with a limit of two per program. Withdrawals are defined as officially withdrawing from any class after the Drop/Add period and before the final withdrawal date as outline din the academic calendar. Any student withdrawing in excess of stated policy may be dismissed from the college of nursing, unless the college of nursing has pre-approved a documented medical and/or emergent situation.

    INCOMPLETE POLICY:

    An incomplete may be awarded only when there is a small portion of work remaining. A student unable to finish a course may ask an instructor for a mark of I (Incomplete). Course instructors may approve or deny a student's request.

    You may be granted a mark of “Incomplete” only if

    • you have finished most of the coursework, and

    • you have an acceptable reason for not completing the course, and

    • your standing in the course is satisfactory.

    Students cannot graduate with an “I” mark on their record. They must either complete the course for a passing grade, or allow the Incomplete to lapse to an “F.”

    ACADEMIC HONESTY POLICY:
